Application for SSS UMID Card - My Personal Experience
Hi there!
This would be my first official post, not counting my introductory post just a while ago.
Today I would like to share how I applied for my SSS UMID Card at SSS Naga City Branch.
Since I changed my status already from single to married, I needed a new valid ID reflecting the change in my last name. So I went to SSS on a Friday which was just after the typhoon Glenda to apply for an SSS Unified Multi-Purpose ID or UMID Card (from what I know, they no longer issue the old SSS ID). You have to be an active SSS member with at least one monthly contribution to apply for the UMID Card.
Fortunately, there were only a few people there. I was thinking that maybe there would be many people who want to transact or apply for SSS loan after the typhoon.
Considering the scenario, I asked the SSS representative for an SSS UMID Card application form. Though you can download the form online so you can accomplish it prior to going to the agency. But since there was still no electricity that time in Naga City because of the aftermath of the typhoon, I was not able to download the form online. Another requirement for the ID application is to provide 2 valid IDs (photocopy and original).
I then asked my hubby, who was with me that time for a pen since there were other people who were using the pens provided by the agency. I filled up my form really fast since the information needed were really easy once. It's divided into 6 parts:
Part I - Name
Part II - Facts of Birth
Part III - Demographic Data (your address)
Part IV - Statement of Consent
Part V - Other Enrollee Data (Contact number/s)
Part VI - For SSS use only (An SSS rep will fill this out and not you)
So there, after I filled out the form, I was asked to proceed to Counter 1 or 2 and submit the form along with 2 valid IDs. The representative was a man and he was really nice. While he was reviewing my application, he told me that I was quite lucky that there were very few people that day because on regular days, SSS entertains a lot of people and that the office would be packed that some people would just stand or wait outside. I was definitely lucky that day because that was only one of my spare time to visit or make personal transactions since I was still working full time.
After approving my ID application, the rep filled up Part IV of the form and instructed me to go to the very last counter and record my fingerprints and have my picture taken. I gladly thanked him and went to the last counter. The rep at the counter was more of an IT type of guy and didn't talk much, he just gave instructions and I obliged. He recorded my fingerprints and signature and took my picture. After that, he gave me the stub, which is at the very last part of the form and said that my ID would be sent to my address via mail after 2-4 weeks.
I never expected that applying for an SSS UMID Card was that fast, all in thirty minutes! Well, maybe because there were few people that day but my experience was not a hassle and the SSS reps were nice.
I still haven't received my UMID Card so I'm still waiting for it to arrive this week or maybe next week. I'll update you as soon as I get my hands on my UMID Card and post details on how to activate it. :)

Hello everybody! This is my first post in my very own blog :) (so excited!)
First, I have to introduce myself.
I'm Joyce, nearing a quarter of a century in two years, married and a blessed mother. I recently got separated from the employed world so I can focus more on taking care of my family and learn more about myself, the married life, motherhood and a whole lot of things that God has planned for my life.
I think I don't have to go into too much details of my personal life for now, just that I am really excited to start writing away and share my experiences with you guys :)
Please bear with me as I am a newbie in blogging. Feel free to suggest any ideas for my blog, I will be open to those! :) But please refrain from posting negative comments about my posts. Let's all respect each and everybody's opinions and thoughts... PLEASE!
